Doctors have long advocated aggressive treatment of immunologic implantation dysfunction in women undergoing IVF. In cases where there has been Natural Killer cell (NKC) activation IVIG has been used to deactivate the NKC. IVIG is very expensive.
Recently Intralipid has been used to deactivate NKC.
Intralipid is a synthetic product composedof 10%soybean oil, 1.2% egg yolk phospholipids, 2.25% glycerin and water.
Infusion of intralipid lowers Natural Killer cell activation as effectively as does intravenous gammaglobulin (IVIG)
When indicated Intralipid is given intravenously 7-14 days prior to Embryo transfer and again after a positive pregnancy test.
An additional treatment is given when a heart beat is seen.
Since 2009 I have been evaluating the effect of Intralipid in patients who had activated Natural Killer cells.
Thus far 1800 women have been treated with 20% Intralipid. More than 60% of patients achieved viable pregnancies.
Intralipid is as effective as IVIG. There were no significant side effects and patient tolerance to treatment was high.
